Outreach at Sage is available when you need help with more than one area of your life. Maybe you are having financial difficulties and problems with housing. Or perhaps you have physical health issues, problems with your family, and feelings of loneliness. Social workers at Sage can help you deal with all areas of your life...so you can be happier, healthier, and better connected with the supports that are available!

Our Outreach program can help you in many ways:

1. Identify your needs: financial, housing, legal, abuse, income tax, physical health, mental health, mobility, transportation, family dynamics, home service and repair, food, and more.

2. Connect you with resources: Social workers are knowledgeable on senior-specific resources and will connect you with other programs and services that may help you.

3. Emotional support: Social workers value and support you without judgment.

4. Community outreach: If you are unable to come into Sage, our social workers can meet you in your Edmonton area home.

Sometimes people need longer support to deal with a number of complex issues and we will support you to work through those issues. This may include providing support to you when attending meetings with your doctor during important appointments, to help you complete government forms, and act on your behalf to report your needs to others.

You can also call 780-423-5510, extension 323 or 587-773-1764 to leave a voicemail, and one of our social workers will return your call within 2 business days. Home visits can also be arranged.
