INTENSIVE CASE MANAGEMENT COORDINATOR, SAFE HOUSE

As one of two Intensive Case Management Coordinators, this position is responsible for providing comprehensive, holistic services to older adults experiencing elder abuse in the community of Edmonton who may or may not be admitted to the Safe House shelter. This includes assessing risk and needs, providing information, support and client advocacy while assisting them to achieve safety. This position will work in collaboration with the Sage team including the Safe House and community service providers to best meet the specialized needs of each individual. 

QUALIFICATIONS, SKILLS and EXPERIENCE

  • Social Work degree or diploma
  • Registered Social Worker with the ACSW is required
  • Minimum of 2-3 years of Social Work case management experience preferred
  • Access to a reliable vehicle with $2 million in liability insurance; valid Class 5 driver’s license
  • Undergo Police Information Check with Vulnerable Sector search
  • Experience working with older adults and/or victims of abuse
  • Knowledge of community resources and services related to older people and care providers
  • Excellent interview, assessment and communication skills

COMPENSATION PACKAGE 

This position is a permanent full-time career opportunity. The starting salary range is between $42,448 - $51,203 plus 100% Employer paid Health & Dental plan, 5% matched Pension plan, re-imbursed ACSW registration fees and Social Work liability premiums. Salary considerations are based on assessment of the successful candidate’s education, skills, previous experience and other factors. Sage offers an outstanding work life balance with a 35-hour work week, 3 weeks of vacation to start plus the last week of December off with pay, and generous sick leave accruals. Sage also observes 13 statutory holidays in the calendar year. Sage is offering great flexibility with the option of a compressed work week. HOUSING NAVIGATION SPECIALIST, SOCIAL PRESCRIBING PROGRAM

The Housing Navigation Specialist will work closely with the Social Prescribing program, and its partners, to bridge the gap between affordable housing collaborations in Edmonton and CBSS organizations. They will also work at a system level with all stakeholders to understand the gaps, needs and barriers for older adults and advocate and develop a plan to improve equitable and timely access to appropriate housing for older adults, some of whom may be experiencing mental health issues, addictions and hoarding disorder. This position also includes data collection and evaluation.

QUALIFICATIONS, SKILLS & EXPERIENCE REQUIRED

  • Degree in Social Sciences
  • Master's Degree is an asset
  • Provide Police Information Check including Vulnerable Sector
  • Relevant work experience in a similar field is required; experience in advocacy and system change work is an asset
  • Vehicle required; valid Class 5 driver’s license and $2 million auto liability insurance
  • Regulated professionals must have active registration in good standing with their respective regulatory bodies
  • Experience with public policy and/or advocacy and/or program development is an asset
  • Have a clear understanding of inclusive, anti-oppressive practice

LINK WORKER, SOCIAL PRESCRIBING PROGRAM

The Link Worker will work with Individuals referred by medical professionals to the Social Prescribing to provide support through connecting older adults to daily living and social support in the community. This could include assistance in accessing food resources, transportation, physical activities, volunteer opportunities or assistance in filling out forms. The Link worker will provide direct support by providing information or attending appointments with the client. The goal of this position is to assist older adults to age safely in place.

QUALIFICATIONS, SKILLS & EXPERIENCE

  • Knowledge of issues impacting older adults and the community resources available
  • Proficient problem-solving skills and the ability to make informed decisions
  • Computer literacy including Microsoft Office
  • Proficient oral and written communication skills and organizational skills
  • Police Information Check including Vulnerable Sector is required
  • Vehicle required; valid class 5 driver’s license and $2 million auto liability insurance
  • Experience working with older adults and volunteers
  • Experience working with diverse cultures

  • Registered Social Worker with ACSW
  • Relevant degree or diploma; Bachelor of Social Work is an asset
  • Experience in community-based case management
  • Strong assessment, interpersonal, problem-solving and decision-making skills required
  • Self-motivated; able to work independently and as part of a team
