Follow up Coordinator (Temporary, Part-Time)

We are looking for an experienced and passionate Follow up Coordinator. This position is responsible for providing follow up services to seniors recently discharged from the Sage Seniors’ Safe House program. This includes assessing needs, providing information and support and client advocacy while assisting them in becoming familiar and safe in their new community. The position will work with Safe House staff to become familiar with the intake and discharge process and discharge plans.

The successful candidate will hold a degree or diploma in Social Work. Experience working with older adults and/or victims of abuse is required. The position requires the ability to become registered with the Alberta College of Social Workers and the use of a vehicle.

The position is a part time (20 hours/week) temporary job opportunity until April 5, 2019.

Assessment, Housing and Outreach Coordinator (Temporary Full-time)

This position provides assessment, information, referral, support, and assistance to older adults addressing basic needs as well as social and emotional needs as identified by the individual senior. This includes assisting the older adult in becoming familiar with housing options, support when making a decision to downsize homes, and providing referrals in regards to landlord tenancy issues. Other tasks include helping older adults prioritize their needs and make informed referrals as necessary, as well as responding to calls from families of older adults, community members, and professionals regarding available resources.
